Education and Experience Current Wisconsin State LPN license.
Current CPR Certification.
A minimum of five years of long-term care experience is preferred, but not required.
Characteristic Duties and Responsibilities Practices high standards of nursing care and implement the policies and procedures of the nursing department.
Provides resident-directed services that honor individual lifestyles and preferences.
Promotes that Colfax Health and Rehabilitation Center is home for the resident.
Consults with attending physicians for treatments, medications, rehabilitation, etc.
Notifies attending physicians, family members, and members of the team when there is a change in the resident's condition.
Completes medication and treatment passes in accordance with state regulations and facility policy.
Completes medical forms, reports, evaluations, studies, and chartings.
Provides leadership with coaching and mentoring other caregivers.
Provides supervision for the nursing assistant's daily tasks and routine care of residents.
Protects residents by keeping patient information confidential and understands Residents' Rights.
Maintains harmonious relationships with co-workers, providing support and assistance to co-workers to ensure quality resident care.
Assists nursing assistant staff in providing direct care when needed.
Actively assists nursing assistants to answer resident-initiated call light services requested as well as alarms.
Creates and maintains an atmosphere of personal interest and a positive emphasis on a calm environment throughout the facility.
Participates in creating an environment on the wing where the residents have ongoing opportunities to participate in individualized and purposeful activities, resembling their previous home environment, lifestyle, and customary daily routine.
Provides and receives reports on resident's condition and status and at shift change.
Actively participates in care conferences when needed, nurse's meetings, and other meetings as required.
Responds promptly to changes in workload, adjusting assignments to achieve optimal productivity and efficiency.
Initiates and retains the resident's clinical records in accordance with the facility procedures.
Makes informative entries in the progress notes that reflect the care provided, and the resident's response to that care.
Greets newly admitted residents upon admission to their wing, and orients new residents and their families to the facility.
Meets and positively interacts with residents and their family members.
Acts as an emergency responder with AED and emergency supplies for needs in the facility.
Manages resident emergencies, assessments of resident conditions, and completion of quality assurance reports as needed.
Completes requirements for proper admission, transfers, and discharges of residents.
Reviews and completes incident reports and conducts investigations as required.
Follows established safety and infection control policies and procedures.
Participates in procedures for reporting hazardous conditions or equipment.
Ensures that all resident care equipment is used and operated in a safe manner Reports all malfunctioning equipment/safety hazards immediately to maintenance.
Initiates the process of dealing with a staff member's call-in according to the facility procedure regarding sick staff.
Is responsible for the house during times when the scheduler is not available to take call-ins.
Understands and acknowledges the responsibilities of the assisted living residents and assisted living staff.
